Collaborative Care: Practice Advice for RNs and NPs

This practice advice provides guidance to registrants  practising in diverse roles and settings and working in collaboration with clients, their caregivers and other members of the health-care team to provide effective, high-quality care through high-functioning collaborative teams. 

The purpose of this guidance is to support registrants in applying the guiding principles of collaborative care: a person-centred approach, team communication, role clarification, team functioning, collaborative leadership and addressing team differences or disagreements.
